Student Observers

Before your first day observing at HeathQuest you will need to print off and sign the Student Observer HIPPA (Privacy Act) Release Form as well as view the PowerPoint presentation of the HIPPA Training Module (see below) and complete the quiz on the information in the PowerPoint. Dress should be a collar shirt with slacks (no jeans or shorts) and comfortable shoes (no flip flops or sandals). Students coming from school may use the locker rooms to change. It is also a good idea to keep a log of what you see while at HealthQuest so you can recall the information when needed.
